The deterioration in plant condition will almost inevitably start the moment it is put into service. So the safe and reliable management of plant is about the overall process and system you have in place to monitor and manage that deterioration through life – and so ensure that your plant is demonstrably fit for service at all times it is in operation.

To understand the potential failure modes, it is necessary to identify and understand the relevant factors from plant design through to the day it is decommissioned.

In many cases, the correct analysis of small problems or failures can assist in the prevention of large or catastrophic failures.

This presentation is based on 35 years of equipment failure investigation

  • Examples are used to illustrate a holistic approach to failure analysis
  • The importance and inter-relationship of data from design, fabrication and operation / maintenance is described
  • Discussion and conclusion are drawn for root cause analysis and effective application of the results
