The American Author, Grace Paley is quoted to have said:
“Hindsight, usually looked down upon, is probably as valuable as foresight, since it does include a few facts.”

The RAIB makes its recommendations to improve railway safety based upon facts, supported by evidence it finds during its investigations. The investigation approach involves analysis of the particular factors which came together to cause a particular accident.
Its investigation also involves uncovering any underlying factors which were present, and, if removed or prevented, are likely to minimise the occurrence of a similar accident. Behind many causal and underlying factors are precursor events, as well as these factors being possible precursors to the accident themselves.

This lecture will explain the RAIB's investigation process and will highlight some precursor events based upon previous railway accident investigations.
